If you asked most dentists, dental nurses, or practice managers what a dental laboratory does, the answer would probably be fairly straightforward. "We send impressions, scans, and prescriptions. The laboratory makes the work. We fit it." Whilst that is technically correct, it only scratches the surface of what happens behind the scenes. From our experience at Bremadent Dental Laboratory, many dental professionals are surprised when they discover how a modern dental laboratory actually operates as a business and production environment. In fact, one of the biggest misconceptions we encounter is that laboratories primarily work for one practice at a time, or that the workload arriving each day is predictable and controlled. The reality is very different. Understanding how a dental laboratory functions helps explain why communication, planning, realistic turnaround times, and strong partnerships are so important within dentistry. A Dental Laboratory Doesn't Have a Patient Appointment Book
One of the easiest ways to understand the challenge is to compare a dental laboratory with a dental practice. Imagine a practice suddenly had 100 patients arrive at reception at 9am without appointments. It would be impossible to see everyone. Patients would need to be booked onto different days, different clinicians would need to be allocated, and workloads would need to be managed. Dental practices control capacity through appointment books. Dental laboratories do not have that luxury. A laboratory cannot tell a case to come back tomorrow. Cases arrive when they arrive. Some days we may receive 50 cases. Some days we may receive 150. Some days we may receive large complex implant cases, multiple full dentures, urgent repairs, orthodontic appliances, crowns, bridges, and digital workflows all at once. Unlike patient appointments, laboratory workloads are often unpredictable from one day to the next. That creates a unique challenge that many people outside laboratory dentistry never see. Every Case Has Multiple Stages Another common misconception is that a denture, crown, or appliance is simply "made" in a few hours. In reality, every restoration goes through multiple technical stages. Take a full upper and lower denture as an example. The process may include: • Case assessment and prescription review • Model preparation • Bite registration analysis • Tooth setup • Wax trial preparation • Investment • Processing • Polymerisation • Deflasking • Trimming • Finishing • Polishing • Quality control • Packing • Invoicing • Dispatch Many of these stages involve waiting periods, curing cycles, material setting times, and overnight processing. Even if a technician worked continuously, some procedures physically cannot be compressed into a few hours because materials require time to perform correctly. A denture processed too quickly may compromise fit, aesthetics, strength, or longevity. Good dentistry often requires patience. Technology Helps, But It Doesn't Eliminate Time Digital dentistry has transformed our profession. Intraoral scanners, CAD software, 3D printing, milling machines, AI-assisted workflows, and digital smile design have significantly improved efficiency. However, technology does not magically eliminate production time. A digital scan still requires design. A designed restoration still requires manufacture. Manufactured work still requires finishing, verification, quality control, and delivery. Technology improves consistency and productivity, but it does not remove the need for skilled technicians. Behind every successful digital workflow is a trained human making important decisions. The Economics of Dental Laboratories Are Very Different One area that is rarely discussed openly is pricing. Dental laboratories operate within a very different financial model compared to dental practices. Let's use a simplified example. At the time of writing, NHS Band 3 treatment attracts a patient charge of over £300. Many full dentures supplied through NHS channels generate laboratory fees that are only a fraction of that figure. The laboratory still has substantial costs to cover, including: • Materials • Teeth and acrylics • Digital software licences • CAD design systems • 3D printers • Milling equipment • Scanner maintenance • Staff wages • Administration teams • Quality control • Delivery services • Utilities • Compliance costs • Training • Equipment replacement Laboratories also absorb many hidden costs that are not always visible from outside the business. For example, a delivery driver collecting and delivering cases may visit dozens of practices each day. Cases may require remakes. Materials may increase in price. Equipment may require servicing. Compliance requirements continue to evolve. All of these costs must be funded whilst maintaining quality and service levels. Why Volume Matters Because laboratory fees are often relatively modest compared to the total treatment value, laboratories typically rely on volume to remain commercially viable. This is where another misconception sometimes arises. Practices may wonder why laboratories encourage consolidation of work or long-term partnerships. The answer is simple. Volume creates stability. Stable workflows allow laboratories to invest in better equipment, better staff training, improved quality systems, and more predictable turnaround times. Without sufficient volume, maintaining those investments becomes significantly more difficult. A strong partnership benefits both parties. The practice receives consistency and support. The laboratory gains workflow predictability. Ultimately, the patient benefits. Every Case Is Unique Unlike many manufacturing industries, dentistry is highly customised. A car factory may produce thousands of identical components. A dental laboratory rarely produces two identical cases. Every patient has different anatomy. Every clinician has different preferences. Every restoration carries unique clinical requirements. Even when two cases appear similar on paper, the technical challenges can be completely different. That level of customisation requires skilled technicians who combine science, engineering, artistry, and problem-solving every day. Communication Makes a Huge Difference One of the biggest factors influencing laboratory efficiency is communication.
The best results usually come from practices and laboratories working as one team rather than separate businesses. The Patient Never Sees Most of the Work Perhaps the most fascinating aspect of dental laboratories is that patients rarely see what happens behind the scenes.
