The Gold Standard of Full Metal Crowns: Why They Continue to Earn Their Place in Modern Dentistry

Every few years, dentistry welcomes a new restorative material that promises greater strength, improved aesthetics or enhanced longevity. Zirconia has transformed posterior restorations, lithium disilicate has redefined cosmetic dentistry, and digital workflows have changed the way laboratories manufacture crowns. Yet despite all these advances, one restoration continues to prove its worth decade after decade: the full metal crown.

At Bremadent Dental Laboratory in London, we still manufacture a significant number of full metal crowns every month. While all-ceramic restorations dominate many treatment plans, we continue to receive prescriptions from dentists who specifically request full metal crowns for posterior teeth because they know they remain one of the most predictable restorations available. They are not choosing them because they are old-fashioned. They are choosing them because they work.

Interestingly, we have also noticed an increase in requests for highly polished anterior full metal crowns. Popular culture and the rise of permanent metallic smiles inspired by grills have made these restorations increasingly appealing to some patients. Unlike removable cosmetic grills, however, a full metal crown is a professionally manufactured, permanently cemented restoration that restores the tooth while creating the desired metallic appearance. Most patients are surprised to learn these crowns are often silver in colour rather than gold, with the final appearance created by polishing the alloy to a brilliant mirror finish.

At Bremadent, we manufacture both full metal crowns and cast gold crowns, allowing clinicians to choose the material that best suits each patient's clinical and aesthetic requirements.
Why Full Metal Crowns Continue to Be the Gold Standard
There is a reason many experienced dentists still refer to the full metal crown as the gold standard.
It combines outstanding strength, excellent marginal integrity, conservative tooth preparation and exceptional longevity into one restoration.

Unlike porcelain restorations, there is no ceramic layer to chip.

Unlike layered zirconia, there is no veneering porcelain to fracture.

Unlike many aesthetic materials, metal performs exceptionally well even at reduced thicknesses, allowing significantly more natural tooth structure to be preserved.

Modern restorative dentistry increasingly focuses on minimally invasive treatment. Preserving healthy enamel and dentine whenever possible is fundamental to long-term tooth survival. Because metal possesses excellent mechanical strength, clinicians can prepare teeth more conservatively than they often can for ceramic restorations.
Sometimes the strongest restoration is also the most conservative.

Clinical studies have consistently demonstrated excellent long-term survival rates for full metal crowns, with many remaining functional for several decades when supported by good oral hygiene and appropriate occlusion. That level of predictability explains why many dentists would still choose a full metal crown for their own heavily loaded molars.

The Properties That Make Full Metal Crowns Exceptional
Every restorative material has strengths and limitations. Full metal crowns have remained successful because their physical properties directly solve many of the challenges encountered in everyday restorative dentistry.

Their advantages include:
• Exceptional fracture resistance under heavy occlusal loading
• Outstanding wear resistance
• Excellent marginal adaptation
• Minimal tooth reduction
• High dimensional stability
• Long clinical lifespan
• Smooth polished surfaces that are kind to opposing dentition
• Excellent performance for patients with parafunctional habits such as bruxism

Posterior teeth are exposed to enormous functional forces every day. During mastication, bite forces commonly exceed several hundred newtons, with considerably higher forces generated in patients who grind or clench their teeth. In these situations, strength becomes more important than translucency.

A properly manufactured full metal crown is capable of withstanding these demanding conditions year after year with remarkably little maintenance.

That reliability is why it continues to occupy such an important place in restorative dentistry.

Precision Casting Still Matters
Digital dentistry has undoubtedly improved consistency and communication between practices and laboratories. However, producing an exceptional cast metal restoration still relies on meticulous manufacturing principles and skilled technicians.

At Bremadent Dental Laboratory, every full metal crown and cast gold crown is manufactured entirely in-house within our own quality-controlled laboratory. We do not outsource our cast metal work. By controlling every stage of production ourselves, we maintain complete oversight of quality from the initial wax pattern through to the final high polish.

Before casting even begins, every case benefits from modern digital technology. Wherever appropriate, our full metal crowns are first designed using CAD software, allowing us to create precise anatomical contours, contact points and occlusion within a fully digital workflow. Rather than hand waxing every restoration, the approved design is milled from a high-precision wax blank, producing an exceptionally accurate wax pattern ready for investing.

This digital approach combines the repeatability and accuracy of CAD/CAM technology with the proven reliability of traditional lost-wax casting. Once the wax pattern has been verified, it is carefully sprued, invested and cast under our controlled manufacturing protocols. By combining digital design with precision casting, we achieve highly consistent restorations with excellent fit, predictable occlusion and reduced variability compared with conventional hand-waxed techniques alone.

Many people assume casting is simply melting metal and pouring it into a mould. In reality, every stage influences the final accuracy, fit and longevity of the restoration.

It begins with the investment material. The powder-to-water ratio is accurately measured for every mix to achieve predictable setting expansion and minimise the risk of nodules, bubbles, porosity and inaccuracies within the mould. Even small variations can affect the final casting.

The burnout cycle is equally critical. Our furnaces follow carefully controlled heating programmes that completely eliminate wax while allowing the investment to reach its optimum casting condition. Controlled heating also helps minimise unnecessary stresses that could compromise accuracy.

Metal melting and casting are performed under carefully monitored conditions using calibrated equipment appropriate for each alloy. Temperature control is essential. Overheating can alter alloy characteristics, while insufficient temperature may result in incomplete casting or poor marginal adaptation. By carefully controlling these variables, we maximise the physical properties of each alloy and achieve consistently accurate restorations.

Cooling is another stage that often receives little attention but has a significant influence on the finished restoration. Controlled cooling protocols help reduce internal stresses and minimise distortion before the restoration proceeds to finishing and polishing.

By controlling every stage of the manufacturing process, from investment mixing and burnout through to casting, cooling and finishing, we reduce the risk of distortion, uncontrolled expansion, bubbles, porosity and internal stresses that can compromise both fit and longevity.

The result is a restoration with excellent marginal integrity, predictable seating and fewer chairside adjustments.
For clinicians, that means greater confidence when cementing the restoration. For patients, it means a restoration designed to provide many years of reliable service. Precision casting remains every bit as much a science as it is a craft.

When Should Dentists Consider a Full Metal Crown?
Although aesthetics dominate many restorative discussions today, function should always remain the primary consideration.
Full metal crowns remain an excellent option for:
• First and second molars
• Patients with heavy occlusion
• Bruxists
• Limited occlusal clearance
• Endodontically treated posterior teeth
• Patients prioritising longevity over aesthetics
• Situations requiring conservative tooth preparation
• Long-term definitive restorations
Every case should be assessed individually, but when function, durability and predictability take priority, full metal crowns continue to outperform many modern alternatives.

The Growing Popularity of Metallic Smiles
One fascinating trend we have observed over recent years is the increasing number of patients requesting visible anterior metal crowns.

This is not simply about restoring damaged teeth. Fashion has played a role.
The popularity of grills across music, entertainment and social media has introduced many younger patients to metallic smiles. Some now request permanently cemented restorations that deliver the same appearance while functioning as genuine dental restorations.

It is important to distinguish these from cosmetic grills. A full metal crown is a clinically prescribed restoration that restores strength, protects the underlying tooth and is permanently cemented.

Most full metal crowns are manufactured from silver-coloured cobalt chromium alloys before being polished to an exceptional mirror finish. While yellow gold remains available, many patients actually prefer the bright silver appearance.

Like every treatment, careful case selection and informed consent remain essential, particularly where anterior aesthetics are involved.

Although cobalt chromium remains a popular choice, cast gold crowns continue to represent one of the finest restorative materials available.

A well-manufactured cast gold crown can provide decades of predictable function. That is difficult to ignore.

Communication Between Practice and Laboratory Matters
Producing an outstanding full metal crown depends upon more than technical skill. Good communication between the dentist and laboratory remains essential.

Providing clear prescriptions, accurate occlusal information, appropriate preparation design and digital scans or quality impressions enables technicians to manufacture restorations that require minimal adjustment at fit.

Helpful information includes:

• Occlusal clearance
• Opposing dentition
• Bruxism history
• Material preference
• Cementation requirements
• Clinical photographs where appropriate

Small details supplied at the prescription stage frequently save valuable chairside time later.
