To make a claim from the warranty participating clients will need to submit a claim form along with: |

| 1. | A copy of your original laboratory invoice showing the Warranty cover (as a seperate line) is in place for the claim. |
| 2. | Patient details and the reason for the claim |
| 3. | Photograpic evidence to support the claim |
| 4. | An indication of the proposed remedial treatment. |
After returning the form to us we will forward this to the underwriters and then issue you with a cheque on completion of the claim. |
